I guess the best options for non-US players are Click2Pay, EziPay (former PaySpark) and Neteller. Debit/Credit cards are accepted everyhere. UKash and other pre-paid cards can be used to fund the account.
US players can deposit/withdraw money by ECO card - Europe-based company. You have to pay substancial fees for transferring money to ECO card by Western Union or MoneyGram. However, withdrawals are relatively cheap - you pay just $10 for a Bank Wire. Some credit cards are also accepted at several casinos, but most banks block such CC transfers.
